I acknowledge that I have received a copy of the Around The Clock Restaurant & Bakery Employee Handbook dated 2026. I understand that it is my responsibility to read and comply with the policies contained in this handbook and any revisions made to it. I understand that this handbook is not a contract of employment and that my employment is at-will, meaning either the company or I may terminate the employment relationship at any time, with or without cause or notice.
I understand that the company reserves the right to modify, revoke, suspend, terminate, or change any or all policies, procedures, benefits, and rules of conduct, in whole or in part, at any time, with or without prior notice. I understand that no manager, supervisor, or representative of the company, other than the owner, has the authority to enter into any agreement with me for employment for a specified period of time or to make any agreement contrary to the foregoing.

TIP CREDIT ACKNOWLEDGMENT — ILLINOIS
Pursuant to the Illinois Minimum Wage Law, 820 ILCS 105/4(c) and the Wage Payment and Collection Act, 820 ILCS 115/4.1:
As a tipped employee of Around The Clock Restaurant & Bakery, you are hereby notified that your employer intends to claim a tip credit toward the applicable minimum wage obligation. This notice is provided to you in accordance with state and federal law.

Your rights under Illinois law:
Your employer will pay you a direct cash wage of $9.00 per hour.
Your employer claims a tip credit of $6.00 per hour toward the Illinois minimum wage of $15.00 per hour (effective January 1, 2026).
You must retain all tips received, except for a valid tip pooling arrangement limited to employees who customarily and regularly receive tips.
If your tips combined with the cash wage do not equal or exceed $15.00 per hour for any workweek, your employer is legally obligated to pay you the difference.
The tip credit cannot be applied to any overtime hours. Overtime is calculated at 1.5x the full minimum wage.
You may not be required to share tips with non-tipped employees (kitchen staff, dishwashers, management).
Tip Pooling Disclosure: If a tip pooling arrangement is in effect, it applies only among customarily tipped employees (servers, bartenders, bussers, hosts). Management and owners do not participate in the tip pool. The specific terms of the tip pool arrangement are outlined in the Employee Handbook.

By signing below, I acknowledge that I have been informed of the tip credit provisions under Illinois law. I understand the amount of the cash wage I will receive, the amount of tip credit that will be claimed by my employer, and that all tips I receive are my property except for valid tip pooling arrangements. I understand that if my tips combined with my cash wage do not equal or exceed the minimum wage, my employer will make up the difference.

SEXUAL HARASSMENT PREVENTION TRAINING LOG
Illinois SB75 / Public Act 101-0221 — Workplace Transparency Act: All Illinois employers are required to provide sexual harassment prevention training to ALL employees on an annual basis. Restaurants and bars are subject to additional supplemental training requirements under the Hotel and Casino Employee Safety Act (SB0075). Failure to comply may result in civil penalties of up to $5,000 per offense.
